Special Requirements: Travel is required :Travel can be anywhere within King County. The vast majority of travel will be to Queen Anne Healthcare (a nursing home-2717 Dexter Ave N Seattle 98109) but can be at different Adult family homes, residences, shelter, street, wherever the patient resides. I would expect they would travel 2, maybe 3 times per week. Yes, they can all be planned, unless something unexpected comes up. Traveler would need a vehicle. As it may be difficult to get to some AFH via public transit.

This is a full-time FTE under Bed Readiness, Care Management; Monday through Friday, with potential for a pre-planned one weekend day shift per month. The position is for a 2 year long State Pilot program looking at transitioning complex patients from the hospital to the community. Flexibility to work to work in an inpatient, outpatient, post-acute facility and community dwelling is necessary. In addition to hospital experience, community mental health and post-acute setting experience is suggested. EPIC fluency is a strong positive.Facility Name: ***
