APN Healthcare Solutions is urgently hiring multiple LPN - LTC for our client in Dallastown, PA for a 8-week contract with a high possibility of extension.
Position Title: LPN - LTC
Pay Rate: $30/Hourly
Work Location: Dallastown, PA
Shift: 8*5 – (3:00 PM - 11:30 PM)
Guaranteed Hours: 32 Hours
Contract Duration: 8 Weeks
License: State LPN

Job Description:
Responsibilities:
  • Provide skilled nursing care within a long-term care facility, ensuring the comfort and well-being of residents.
  • Administer medications and treatments as prescribed by physicians and document patient responses accurately.
  • Monitor residents' vital signs, assess their health status, and report any changes or concerns to the registered nurse or physician.
  • Assist with activities of daily living such as bathing, dressing, and feeding residents as needed.
  • Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans.
  • Maintain a clean and safe environment for residents, adhering to infection control protocols.
  • Educate residents and their families on healthcare procedures, medication regimens, and disease management.
  • Participate in staff meetings, continuing education programs, and quality improvement initiatives.
  • Demonstrate compassion, empathy, and professionalism in all interactions with residents and their families.
  • Adhere to ethical and legal standards of nursing practice and uphold the facility's policies and procedures.
