What you will be doing:

Under the direction of the Clinical Supervisor of Kids on the Move, evaluates, plans, and administers medically prescribed or educationally relevant physical therapy treatment to clients in their homes or students in their respective educational environment. In addition, this position is responsible for clinical supervision of students, physical therapy assistants and rehabilitation aides.
Obtain and maintain Cardio-Pulmonary Resuscitation CPR certification required.
Working knowledge of medical terminology required.
Effectively operate McKesson software (patient management software) required.
Create and maintain an environment of open two-way communication (with staff) required.
Knowledge of and ability to comply with HIPAA regulations required.
Must be highly organized and able to independently manage time and produce work within required deadlines.
Participate in departmental Quality Improvement activities required.
Effectively and accurately interpret OASIS comprehensive assessment tool required.
Perform OASIS time points in accordance with agency and CMS regulations and policies.
Complete clinical documentation in accordance with agency, State and Federal regulations and policies required.
Consistently display professional, ethical, standards of care consistent with VNAHSR policies and procedures required.
Successfully complete annual Safety Awareness training required.
Display flexibility to work as needed including unplanned schedule changes required.
Knowledge of CMS regulations required.
Ability to adhere to and model agency HEALTH Matters values and Customer Service standards required.
Ability to work nights, weekends, and holidays required.
Evaluate and appropriately plan, implement and document individualized plan of care in accordance with APTA guidelines required.

Experience/Skills you will need:

Successful completion of an accredited Physical Therapy program required.
Two years of clinical Physical Therapy experience preferred.
Ability to obtain valid Vermont State Physical Therapist licensure by date of employment required.
Valid Driver’s license required. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required. Must show proof of auto insurance at time of employment required. Knowledge of the Vermont Therapy Standards required.
Knowledge of State Home Health Rules, and regulations required.
Knowledge of school contract elements required.
