Responsibilities:

The San Mateo County Health System (SMC Health) is searching for a consultant to serve as an
Electronic Health Record (EHR) Change Manager for its EHR Program Office. The Change Manager
will be responsible for completion of the following tasks:

– Planning/Kick-off:
Lead the change activities under the direction of the EHR Program Director and the CMIO, in collaboration with the EHR Project Manager.
Create a detailed change management strategy and/or plan for the design, testing, training and go-live
phases of the project.
Develop and execute communication plan in collaboration with the EHR Project Manager.
Create and assist in presenting the kick-off presentation, including the how the change management
strategy complements the implementation methodology.
Participate and assist the EHR Project Manager in planning the Project Steering Team and related
activities/meetings/communications.

– Design/Build Phases:
Ensure adherence to implementation methodology and coordination of work between team members, third party contractors and vendors to deliver projects according to plan.
Escalate decisions/issues/risks to Project Steering Committee or EHR Program Director/Executive
Steering Committee as appropriate.
Adapt current state workflow design for use in the corrections setting; advise on best practice if existing workflow design is inadequate.
Create or ensure creation of documentation of unique workflows for use in training and testing; adapt and/or build out current training documentation for use in the correctional setting.
Advise and obtain appropriate approval of application content based on gathered requirements, system review and best practices.
Ensure relevant policies and procedures are revised and/or created.
Prepare for integrated testing event; identify, prepare and train testers for testing.
Prepare for training event; identify SuperUsers, create/consolidate training materials, identify and
coordinate staff to be trained.
Assist in identifying, documenting and training for Command Center, SuperUsers floor support, during golive and post go-live

– Integrated Testing:
Conduct integrated testing event according to testing strategy
Assist with workflow/content questions as requested
Assist in logging and resolving testing issues

– Training:
Identify training locations/facilities
Create the training strategy for the new EHR; include go-live/command center process and support
process
Lead the training team/trainers
Oversee the training registration and security access for end-users
Create training materials with the trainers
Execute training

– Go-live Planning and Event:
Communicate and train on go-live/command center process/SuperUser
With the EHR Project Manager, identify SuperUsers and floor support for go-live event; provide schedule by location
Prepare end-users and train on turnover to Support

– Support:
All outstanding issues documented and transitioned to Support
Provide project closure report to ESC and PSC for review and approval

– Other project-related tasks as required
– Once project scope, resources and timeline are defined and agreed upon, delivery of project on time and on Budget

Required Skills:

Bachelors Degree in a health-related field such as Nursing, Pharmacy or Public Health
Project Management Professional (PMP) or would consider equivalent work experience
Change Management experience in the healthcare industry, preferably in the EHR/Provider space
Demonstrated senior-level change management experience
Experience managing change on complex projects
Ability to lead multi-disciplinary teams, facilitate the Project Steering Committee and work collaboratively with resources at various levels within the organization (including executives, C-suite,
providers, nurses, pharmacists and other office staff)
Ability to manage and work collaboratively with various IT groups, vendors and third-party contractors
A strong work ethic and ability to work independently
Excellent prioritization, time management and organizational skills
Excellent verbal and written communication skills
Experience with Microsoft Project, Excel, PowerPoint, Visio and Word
